CEMP Submission Requirements:

✔ AHCA Criteria
✔ Proof of Emergency Status System (ESS) Registration (F.K.A. Florida HealthSTAT)
✔ Proof of Approved Fire Safety Plan and
✔ Current Mutual Aid Agreements for Evacuations.
Note: All criteria is required to be submitted as one PDF Document. See criteria below for your reference.
Note: Emergency Power Plans are required to be submitted along with the CEMP. The new criteria for Nursing Homes and Assisted Living Facilities includes Emergency Power Plan criteria.

Documents will NOT be reviewed if submitted separately.
